New Patient

Here you will find information and forms you will need before becoming a patient of Smith Dermatology. Please print and fill out all four of the patient information forms on the left navigation menu. Please bring the completed forms, a picture ID, and your medical insurance cards with you to your appointment. If you have Medicare, please bring any secondary policy cards, (except Medicaid) with you as well.
